During the administration, the effects of the drug are diminished by these adaptations (tolerance), while the cessation of drug use results in the emergence of these opposing anti-drug responses such as abstinence syndrome. Recent tests on pain responses challenge this simple notion of withdrawal and suggest that opposing drug aversive states can play a more important role in drug dependence than previous thinking. While opioids like heroin produce analgesia, people with a history of opioid self-administration are hypersensitive to certain types of pain during the time they are under the influence of analgesic drugs. This suggests that in pain systems, the anti-drug response exceeds the inhibiting effect of the pain of the same drug. This hyperalgesia is evident in people with a history of heroin use and is not modified by methadone or buprenorphine treatment, but is reduced by long-term withdrawal of opioids. This same pattern of the opposite anti-drug response that exceeds the drug effect can also occur for mood. While the opioids cause the elevation of the mood, commonly described as euphoria and the reduction of emotional distress, participants in the maintenance of the methadone show a significant negative disturbance relative to controls. Thus, for pain and mood, the chronic opioid user under the influence of the medication does not experience an opioid effect decreased by tolerance but rather a state opposed to the effect of the medication. Increases in the concentration of drugs from administration only serve to reduce the degree of pain and disturbance of the mood. These states of aversive pain and mood can contribute to the motivation for the continued use of drugs and the dysfunction associated with drug dependence. Copyright 2004 Elsevier Ltd.
